Elegibility: Firms must be headquartered in the United States or Canada and have a minimum of 10 full-time employees. Additional eligibility requirements can be found in the entry materials.

Participation: All companies must complete the Corporate Survey and get a minimum of 20% of the firm’s employees to complete the Employee Survey.

Deadlines: The deadline for completing the Corporate Survey is April 23, 2010. To continue eligibility, 20% of your firm's employees must participate and complete the Employee Survey by May 21, 2010.

Corporate Application Fee: $200 through March 5, 2010. $250 after March 5. The Corporate Survey must be completed by March 5 to receive the $50 discount. There is no fee for the Employee Survey.
